    目的 探讨电针联合丰富康复训练对磷脂酰肌醇3激酶/蛋白激酶B(phosphatidylinositol 3-kinase/protein kinase,PI3K/AKT)信号通路的影响及血管新生机制。方法 将60只SD大鼠随机分为假手术组、模型组、针康组、抑制剂组,每组15只,参照Longa线栓法建立局灶性大脑中动脉缺血动物模型。针康组于模型复制后给予电针及康复治疗,电针取穴“百会”“风府”“内关”“心俞”,每日1次,连续治疗7 d。抑制剂组侧脑室注射AKT阻断剂后,进行电针联合康复训练干预,每日1次,连续治疗7 d。干预结束后,比较各组大鼠神经功能评分、脑梗死面积,分别采用免疫组织化学法和Western blot法检测缺血侧脑组织中PI3K、AKT蛋白的表达水平。结果 与假手术组比较,模型组神经功能缺损评分显著增加(P<0.05),梗死面积显著增大(P<0.05),缺血侧脑组织中PI3K、AKT蛋白表达水平显著升高(P<0.05);与模型组比较,针康组神经功能缺损评分显著降低(P<0.05),脑梗死面积显著缩小(P<0.05),缺血侧脑组织PI3K、AKT蛋白表达水平显著增加(P<0.05);与针康组比较,抑制剂组大鼠神经功能缺损评分显著增加(P<0.05),脑梗死面积显著增大(P<0.05),缺血侧脑组织PI3K、AKT蛋白表达水平显著降低(P<0.05)。结论 电针联合丰富康复训练可有效促进脑缺血大鼠神经功能恢复,减小脑梗死面积,这可能与其激活PI3K/AKT信号通路,诱导血管新生有关。

    Abstract:

    Objective To investigate the effect of electroacupuncture combined with enriched rehabilitation training on the phosphatidylinositol 3-kinase/protein kinase (PI3K/AKT) signaling pathway and the mechanism of angiogenesis. Methods A total of 60 Sprague-Dawley rats were randomly divided into sham-operation group, model group, electroacupuncture+rehabilitation group, and inhibitor group, with 15 rats in each group, and the Longa suture method was used to establish a model of focal middle cerebral artery ischemia. The rats in the electroacupuncture+rehabilitation group were given electroacupuncture (at Baihui, Fengfu, Neiguan, and Xinshu) and rehabilitation treatment once a day for 7 consecutive days, and those in the inhibitor group were given intracerebroventricular injection of AKT antagonist, followed by electroacupuncture combined with rehabilitation treatment once a day for 7 consecutive days. After intervention, the groups were compared in terms of neurologic impairment score, cerebral infarct area. The expression levels of PI3K and AKT proteins in the ischemic side of the brain were assayed using immunohistochemical method and Western blot method, respectively. Results Compared with the sham-operation group, the model group had significant neurological impairment (P<0.05), a significantly larger infarct area (P<0.05), and significant increases in expression levels of PI3K and AKT proteins in the ischemic side of the brain (P<0.05). Compared with the model group, the electroacupuncture+rehabilitation group had significant reductions in neurologic impairment score and cerebral infarct area (P<0.05) and significant increases in expression levels of PI3K and AKT proteins in the ischemic side of the brain (P<0.05). Compared with the electroacupuncture+rehabilitation group, the inhibitor group had significant increases in neurologic impairment score and cerebral infarct area (P<0.05) and significant reductions in expression levels of PI3K and AKT proteins in the ischemic side of the brain (P<0.05). Conclusion Electroacupuncture combined with enriched rehabilitation training can effectively promote neurological function recovery and reduce cerebral infarct area in rats with cerebral ischemia, possibly by activating the PI3K/AKT signaling pathway and inducing angiogenesis.
